Doctorate of Physical Therapy
Application for Admission

Physical Therapy applicants must apply through Physical Therapist Centralized Application Service (PTCAS) which allows PT applicants to use a single, web-based application and one set of materials to apply to multiple PT programs. PTCAS is a service of the American Physical Therapy Association (APTA) and is administered by Liaison International (LI), an education information technology company located in Watertown, Massachusetts.

It is the applicant's responsibility to read and follow PTCAS and program-specific instructions.

DPT Supplemental Form

As part of the admission application process through PTCAS,  the DPT Supplemental Form is required for North Georgia College & State University.  Applicants should complete the supplemental form by the application deadlines - August 15, 2011, for Early Decision, or December 15, 2011.

DPT Immunization Form

This form should not be completed more than 3 months prior to the start of classes as the PPD will expire if administered sooner.
